The Medical Director, Transition Care is responsible for the post acute care period for OptumHealth patients and coordinates / administers the clinical management function for the assigned health plan patients discharged from a skilled nursing facility, to a clinic/home setting. The physician provides clinical care expertise while working in conjunction with the medical and case management staff and the Medical Director. These responsibilities will include medical management and quality care for the assigned patients. Review with the hospitalists and SNF clinical teams the appropriateness of care compared to guidelines, and oversee the clinical workflows and treatment guidelines for the patient post discharge. The role will closely coordinate with the Chronic Clinic and the clinical staff to assist in successful post acute patient management and access to care.
CLINICAL
1. Supervise care of patients upon admission through post 30days discharge.
2. Ensure proper disposition of patients into home, skilled nursing, hospice, homecare, and home monitoring programs.
3. Establish clinical workflows and oversight of the home monitoring program for patients
4. Utilize the services of contracted providers including physicians and ancillary services as appropriate
5. See patients and/or follow up patients in the Chronic clinic as determined necessary by or in collaboration with Medical Director
ADMINISTRATIVE
1. Communicate clearly and completely with PCP network physicians and other professional staff concerning care issues
2. Attend operational and administrative meetings as requested by the Medical Director or his/her designee
3. Maintain hospital privileges in good standing
4. Collaborate to affect the delivery of timely and appropriate medical care and services including developing strong working relationships with hospital ER and Hospitalist clinical teams.
5. Provide support to all members of the case management team.
6. Strive to continually improve team efficiencies and patient outcomes
7. Uphold team goals by acknowledging and owning personal accountabilities
8. Commit oneself to continued support and accountability to Optum's program.
9 Assist in business development as necessary.
Qualifications
Skills:
  • Possession of successful management, problem solving, and team building skills required.
  • Computer literacy, strong verbal and written communication skills required.
  • Strong clinical skills and competency required, including understanding of nursing homes, assisted living, and home care.
  • Knowledge of managed care and utilization management principles required.
  • Experience: Minimum two (2) years clinical experience required. Experience in Hospice Care.
  • Current active unrestricted Arizona medical licensure in applicable specialty required. Board certified/eligible MD or DO required.
  • Current Arizona Medical licensure in applicable specialty.
  • Maintenance of active privileges with no restrictions at hospitals, and or skilled nursing facilities where Optum patients are located.
